Webpage Request Form

This form enables the user to send in a request form for a webpage containing information for a committee event or sign up.

Please use this form to make your request for a PTA-related activity, event or sign up to have a webpage on the Chat PTA website. You also need to submit a separate request to be included in the Weekly Chat. The Weekly Chat is the PTA's main form of communicating events in a weekly email blast to the parent and caregiver population at Chatsworth. The information may also be included on the PTA Homepage that week.

All webpage requests must be submitted 2 weeks prior to when you want the webpage live.

Sign Up/Registration info that needs to be included
1)target grades, # limit of registrations and if a wait list is needed. 2)what fields are required, ie-name,grade,parent info,email,etc...

We will make every attempt possible to promote your event. Please note that the PTA executive committee may make changes in wording, to timing of placement and duration of message appearing on website.
