Online enrollment form

 Printable version

 Print application only

Dear Parents,

We are pleased to present the registration packet for the Murray Avenue School PTA Spring 2012 After-School Clubs (ASCs).

1) ENROLLMENT PROCEDURE:

2) LOTTERY SYSTEM:

·         Any club that receives more applications than the maximum number of children allowed will go to a lottery (random drawing).  Only applications received by Monday, February 13th, will be eligible for the lottery.

·         Also, please indicate an alternate club whenever possible.  Not listing an alternate club does not exempt your child’s application from a lottery.  Additional club requests will not be honored after the lottery process.

3) NOTIFICATION SYSTEM:  Assume your child is in his/her club(s) of choice unless you are notified otherwise on or before Wednesday, February 29th.  Club assignments will be posted on the ASC bulletin board (opposite the library) on Wednesday,

February 29th.   Clubs begin Monday, March 5th

4) PTA CLUB SCHOLARSHIPS:  Scholarships are available so all children can participate in ASCs regardless of financial situation.  Scholarship requests are strictly confidential.  Please notify Jennifer Monaco as soon as possible to request a scholarship so that your childs club choice can be included in the lottery.  Scholarships are limited to one club per child per club offering.

For more information, please contact Margarita Barrera at 834-5204 (mecbrg@gmail.com), Effie Han at 834-5121 (ehan1@optonline.net), or Karen Belmont at 630-0797 (karen_debruin@hotmail.com) or Kim Vetere at 630-2186 (Rmamaroneck@aol.com)

RULES, REGULATIONS AND PROCEDURES

IF YOUR CHILD ENROLLS IN A CLUB, PLEASE SAVE THIS SHEET FOR REFERENCE

·         General club dates are listed on the attached calendar; however, certain clubs may have dates that vary from the calendar.  Please see individual club descriptions for any variations.  Post club dates on your family calendar.  Please also note the dates that your club(s) will NOT meet.

·         We welcome Kindergarteners to Spring clubs only.

·         Remind your child in the morning if they have a club after school that day. If your child gets picked up from a club, remind your child to stay in the club location until they are picked up.  If you are late picking up your child, the instructor or club aide will bring your child to the main office for late pickup.

·         Students attending clubs assemble in the auditorium immediately after dismissal.  Students sit by club in the auditorium, and instructors meet them there.  Instructors escort students to club locations. Dismissal and pickup are from club locations.

·         Attendance is taken in clubs.  If your child is attending school but will not be attending club, please let club aide Pam Washington at 393-9298, or pwash@optonline.net, know; please do not communicate with your child’s classroom teacher in this regard.  If your child is absent from club and his/her whereabouts cannot be determined, the club aide will call all contact numbers and emergency numbers on file until a parent/caregiver is reached to confirm that the child is safe.

·         If school is in session but after-school activities are cancelled due to inclement weather, you will be contacted by email.  Please make sure your child is picked up directly after school.

·         The nurse is not on duty during club time.

·         Participating in After-School Clubs is a privilege.  All children are expected to behave appropriately in clubs.  Any child who compromises the safety or enjoyment of the experience for others, or is disrespectful to the instructor or other club participants, may be removed from the club with no refund of the club fee.

·         Clubs end at 4:05pm (3:35pm on Wednesdays).  Unless you have explicitly given permission on the enrollment form for your child to leave on his/her own, you are required to pick up your child at the appropriate room on time.  The instructor will bring children that have not been picked up on time to the office.  Please consider the fact that school staff may not be available after the office closes at 4 pm as well as the fact that crossing guards are not on duty at club dismissal time in determining whether your child is able to be dismissed on his/her own.

·         A child may be picked up only by those designated on the application form. If alternate arrangements are made (such as play dates), then these must be communicated in writing to the club instructor.

·         If a parent/caregiver is consistently late in picking up from club, that child could be removed from the club with no refund.

·         For security, two entrances will be open for club pick-up:  the main front entrance on Murray Avenue, and the entrance by Ms. Melnyk’s room on Daymon Terrace, which is the door closest to the large playground on Daymon Terrace.

(Registration deadline:  Monday, February 13th.  Clubs begin on March 5th.)
