After School Clubs
Registration for spring 2012 after school clubs is now open!

Registering for a club(s)
If you wish to apply for more than one club for your child, or to apply on behalf of more than one child, please submit a separate form for each child and each club. Attach a separate check, made out to “Chatsworth Avenue PTA”, for each child, each class and each alternate (e.g. you should submit two checks; one for the 1st choice and another for the alternate). Be sure to write your child’s full name and the selected club on the front of each check. Please do NOT staple your check to the form. Unused checks will be destroyed.
Confirmations
You will be notified via e-mail of your child’s club assignment(s) no later than Thursday February 2nd. If you do not receive a confirmation e-mail, please do not send your child to the club.
Refunds
Registration fees are not refundable once the club rosters have been set and you have received your confirmation email from us.
Snack policy
To ensure the safety of all students, no snacks are permitted at after school clubs.
